Job summary

Gentiva is seeking a dedicated Hospice Registered Nurse to serve as RN Case Manager. You will perform comprehensive home-based assessments, deliver skilled nursing interventions, and coordinate care with an interdisciplinary team to ensure compassionate, personalized end‑of‑life support.

You will educate families, document care plans, and participate in on-call activities, while maintaining compliance with regulatory standards and quality initiatives.

Qualifications

• RN license in the state of employment.
• Hospice/palliative knowledge preferred.
• Minimum 1 year RN experience in hospice, home health, or similar setting.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, self-care, and end-of-life needs.
• Maintain timely clinical records and comply with infection control standards.
