Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in Virginia in good standing.
• Minimum 1 year of professional nursing experience within the last 3 years; or a BSN with 2 years’ experience in public health/home care/hospice is preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of nursing process.
• Develop and revise plans of care with physician and interdisciplinary team.
• Initiate plan of care and revise as patient needs change.
• Administer medications and treatments as prescribed by physician.
• Counsel patient and family on nursing and related needs.
• Provide health care instructions per assessment and plan.
• Assist patient with activities of daily living and promote self-sufficiency.
