Job summary

Affinity Hospice Management LLC seeks a compassionate registered nurse to plan, organize and direct hospice care in patients' homes and communities. The RN will conduct comprehensive assessments, develop and revise care plans with patients and families, administer treatments, educate families, and coordinate with a multidisciplinary team to ensure quality end-of-life care.

Strong communication, observation, and nursing skills are essential, with a valid Virginia license.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice (Virginia).
• 1 year of professional nurse experience in last 3 years; or BSN with 2 years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using nursing process.
• Develop and revise plan of care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family in nursing needs and health education.
• Assist patient with activities of daily living and promote self-sufficiency and comfort.
• Maintain accurate clinical notes and communicate with physician.
• Coordinate care with interdisciplinary team and report changes.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise ancillary personnel as needed.
• Engage in quality assessment and performance improvement activities.
