Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing in Virginia.
• Minimum 1 year professional nursing experience in the last 3 years or a Baccalaureate in nursing with 2 years’ experience (preferred).
• No supervisory experience required.
• Background checks and references may be requested.
• Excellent observation, communication, and math skills; nursing skills per competency checklist.
• Must be able to lift and transfer patients; valid driver with insured vehicle.
• Comply with VA state/local/federal reporting requirements.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan with patient and family involvement.
• Administer prescribed medications and treatments.
• Counsel patients and families on nursing and related needs.
• Teach self-care techniques and health instructions to patients and families.
• Maintain safe patient environments and continuity of care.
• Participate in interdisciplinary team meetings and on-call duties.
