Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize and direct hospice care in home and community settings. The role emphasizes patient-centered education and collaboration with families and the hospice interdisciplinary team.

The RN will conduct comprehensive assessments, develop and revise care plans, administer medications, and educate patients and families. On-call duties and driving in Virginia are required to deliver high-quality community health services.

Qualifications

• Graduate of an accredited nursing program.
• RN license in Virginia in good standing.
• 1 year of professional nursing experience in the last 3 years (or 2 years in public health/home care/hospice with a baccalaureate).
• Must be a licensed driver with a valid auto insurance.

Responsibilities

• Conducts initial and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the care plan with patient and family based on nursing diagnosis.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs.
• Communicates with physician and coordinates care with the team.
• Participates in on-call duties and ensures equipment/services are available.
• Supervises ancillary personnel when required.
