Job summary

Affinity Hospice Management LLC seeks a dedicated registered nurse to lead hospice care in home and community settings. The RN will assess patients, develop and adjust plans of care, and educate families in collaboration with physicians and an interdisciplinary team.

The role requires a Virginia RN license, strong communication and problem-solving abilities, and willingness to participate in on-call duties and ongoing education. Travel to patient homes is expected.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in Virginia.
• 1+ year of professional nursing experience within the last 3 years, or BSN with 2+ years in public health/home care/hospice preferred.
• Must be able to lift, transfer patients and be a licensed driver with insured vehicle.

Responsibilities

• Assess and provide comprehensive patient and family assessments to determine hospice needs.
• Deliver professional nursing care using the nursing process and plan of care.
• Write and initiate care plans, regularly re-evaluate patient and family needs.
• Provide patient and family education and counseling on nursing and related needs.
• Maintain accurate clinical notes and communicate with physicians and care team.
• Participate in on-call duties and ensure equipment and supplies are available.
• Supervise hospice aides as required and participate in quality improvement activities.
