Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in Virginia.
• 1–2 years of hospice/home health experience preferred.
• Criminal background check and references required.
• Valid driver’s license with insured vehicle.
• Excellent observation, communication and problem-solving skills.
• Ability to lift and transfer patients and move equipment.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process and plan of care.
• Develop, implement, and revise the care plan based on patient status and needs.
• Deliver appropriate preventive and rehabilitative nursing procedures and administer prescribed treatments.
• Counsel patients and families on nursing needs and health instructions.
• Maintain patient safety and continuity of care with the interdisciplinary team.
• Participate in on-call duties and ensure availability of necessary equipment and services.
• Supervise aides when required and pursue ongoing professional development.
