Job summary

Affinity Hospice Management LLC seeks a dedicated Registered Nurse to lead home-based hospice care in Virginia. The role focuses on comprehensive patient assessments, care planning, and coordinating with families to meet medical and emotional needs.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed as a Registered Nurse in Virginia.
• Experience in home health/hospice or public health is preferred.

Responsibilities

• Performs initial and ongoing comprehensive patient assessments.
• Delivers professional nursing care per the nursing process.
• Develops, implements and revises patient care plans with family involvement.
• Administers prescribed medications and treatments safely.
• Provides patient/family teaching and health counseling.
• Maintains accurate clinical notes on patient status and changes.
• Coordinates care with physicians and the hospice interdisciplinary team.
• Participates in on-call duties and ensures equipment/ supplies are available.
• Maintains safe home environments and supports activities of daily living.
• Follows state and agency requirements and reporting obligations.
