Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and provide clinical, palliative and home-based care to keep patients in their homes as long as possible.

The role requires an active RN license in Florida, at least 1 year of hospice or hospital experience, EMR/EHR proficiency, and a valid driver’s license with reliable transportation. Strong English communication and mobility to visit patients are essential.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Alzheimer's Disease & Related Dementias Training (Florida) if working at PACE.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance; reliable transportation.

Responsibilities

• Assess patient and family needs using the nursing process.
• Coordinate Plan of Care with the Interdisciplinary Team (IDT).
• Provide clinical, palliative and supportive care at home.
• Visit participants in their homes per scheduled visits.
• Maintain documentation and communicate effectively with IDT.
