Job summary

Chapters Health System is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to patients in their home environment.

The role requires an active RN license, a minimum of 1 year nursing experience (hospice or hospital preferred) and proficiency with EMR/EHR systems; a valid driver’s license, reliable transportation, and BLS certification are required.

Qualifications

• RN license active in the practicing state.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• PACE sites may require Alzheimer’s Training per Florida Dept. of Elder Affairs.
• Experience with EMR/EHR systems.
• Valid driver’s license and auto insurance per Company policy.
• Reliable transportation for visit schedule.
• Ability to communicate in English (verbal and written).
• Ability to visit participants at home and perform essential duties (including lifting).
• Active BLS for healthcare professionals.

Responsibilities

• Provides and manages direct care to patients and families as part of the Interdisciplinary Team (IDT).
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patients and families about disease, prevention, palliative interventions and safety practices.
• Home visits to assess safety, medication and nutritional needs for community living.
• Documents changes in patient condition and communicates with the IDT.
• Assists in evaluating hospice referrals/admissions for level of care.
• Presents reports to the IDT; maintains professional communication.
• Supervises Home Health Aides/CHWs as requested.
• May be required to work during emergencies to ensure continuity of services.
• Performs other duties as assigned.
