Job summary

Journey Hospice is seeking a Full-Time RN Case Manager to serve clients in Adams and Franklin Counties. The RN will oversee a primary caseload, perform nursing assessments, and collaborate with the Interdisciplinary Team to develop and adjust the care plan as patient needs change.

Responsibilities include teaching and supervising patients, families, and staff, ensuring adherence to hospice standards and participating in care coordination across the team.

Qualifications

• Graduate of an accredited registered nursing program.
• PA RN Licensure in good standing.
• Valid driver's license and insured transportation.

Responsibilities

• Oversee a primary caseload of Hospice patients and provide skilled nursing care.
• Conduct initial and ongoing comprehensive nursing assessments and update the plan of care.
• Educate, supervise and counsel patients, families, and staff about nursing care and needs.
• Collaborate with the Interdisciplinary Team to ensure patient and family needs are met.
• Supervise Hospice Aides/Homemakers as part of the care team.
