Job summary

Lutheran Social Ministries of New Jersey is seeking a Full-Time RN Case Manager to serve hospice patients in Adams and Franklin Counties. The role requires delivering skilled nursing care, assessments, and developing the hospice plan of care in collaboration with the IDT.

Strong communication with patients and families is essential. Minimum one year general nursing experience; PA RN licensure in good standing and a valid driver's license are required.

Qualifications

• Graduate of an accredited registered nursing program.
• PA RN Licensure in good standing.
• Bachelor’s degree in Nursing preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care.
• Perform initial and ongoing nursing assessments; develop and revise the IDT plan of care.
• Teach, supervise, and counsel patients, families, and staff on nursing care.
• Collaborate with IDT members to meet patient and family care needs.
