Job summary

Journey Hospice, part of Lutheran Social Ministries of New Jersey, is seeking a Full-Time RN Case Manager to serve clients in Adams and Franklin Counties. The role offers a competitive salary and robust benefits.

The Hospice RN will manage a primary caseload, perform initial and ongoing nursing assessments, collaborate with the Interdisciplinary Team to develop and revise the care plan, and provide education and supervision to patients, families, and staff within a compassionate home-care

Qualifications

• Graduate of an accredited registered nursing program.
• PA RN Licensure in good standing.
• Bachelor’s degree in Nursing preferred.
• Valid drivers license and insured vehicle.

Responsibilities

• Oversee primary caseload of Hospice patients and provide nursing care.
• Assess and revise the plan of care with the Interdisciplinary Team (IDT).
• Educate, supervise, and counsel patient, family, and staff.
• Collaborate with IDT to meet patient and family needs.
• Supervise Hospice Aides/Homemakers.
