Job summary

Journey Hospice in Pennsylvania is seeking a full-time RN Case Manager to serve clients in Adams and Franklin Counties. The Hospice RN will oversee a primary caseload and provide skilled nursing care to patients, families, and caregivers as prescribed by the physician and in line with Medicare conditions and agency policies.

Responsibilities include initial and ongoing nursing assessments, developing and implementing the plan of care with the IDT, teaching and supervising staff and caregivers,

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• PA RN Licensure in good standing.
• Valid driver’s license and reliable transportation.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care as prescribed by the physician and agency policies.
• Provide initial and comprehensive nursing assessments and identify problems; update plan of care with IDT.
• Implement and reevaluate the plan of care to meet nursing needs and goals.
• Teach, supervise, and counsel patients, families, and staff on nursing care and patient needs.
• Collaborate with IDT to ensure patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
