Job summary

Journey Hospice in Pennsylvania seeks a Full-Time RN Case Manager to serve clients in Adams and Franklin Counties. The RN will oversee a primary caseload and provide skilled nursing care to patients and families as prescribed by the physician and in compliance with Hospice standards.

Graduate of an accredited RN program, PA RN licensure in good standing, and a valid driver’s license are required. We offer competitive salary, robust benefits, and an equal opportunity employer.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• PA RN Licensure by the PA State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation that is insured in accordance with PA state requirements.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care to them, their families and caregivers as prescribed by the physician and in compliance with Hospice standards.
• Perform the initial and comprehensive nursing assessment and identification of problems; reassess and revise the plan of care with the IDT.
• Teach, supervise, and counsel the patient, family members, and staff regarding nursing care and needs.
• Collaborate with other IDT members to ensure all patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
