Job summary

Journey Hospice has a rewarding and fulfilling position for a Full-Time RN Case Manager to serve our clients in Adams and Franklin Counties. The RN Case Manager will oversee the primary caseload of Hospice patients and provide skilled professional nursing care as prescribed by the physician and in compliance with policies.

The role includes assessing patients, developing and implementing the plan of care, teaching and counseling families, and coordinating with the interdisciplinary team to meet

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• PA RN Licensure by the PA State Board of Nursing in good standing.
• Valid drivers license and reliable transportation that is insured in accordance with PA state requirements.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled professional nursing care.
• Perform initial and comprehensive nursing assessments; develop and revise the plan of care with the IDT.
• Teach, supervise, and counsel patients, families, and caregivers on nursing care and patient needs.
• Collaborate with IDT to ensure all patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
