Job summary

Lutheran Social Ministries of New Jersey seeks a Full-Time RN Case Manager to serve Hospice patients in Adams and Franklin Counties, PA. The RN will conduct initial and ongoing nursing assessments and work with the IDT to implement a comprehensive plan of care.

A valid PA RN license is required, with a Bachelor’s degree preferred. The role emphasizes supervision of Hospice Aides/Homemakers, education for patients and families, and collaboration with the IDT to ensure high-quality end-of-life

Qualifications

• Must have PA RN licensure in good standing.
• Bachelor’s degree in Nursing preferred.

Responsibilities

• Oversee primary caseload of Hospice patients.
• Provide skilled nursing care per physician orders and Medicare/PA guidelines.
• Develop and revise the Hospice IDT plan of care with the team.
• Educate and supervise patients, families, and staff as needed.
• Coordinate with other IDT members to meet patient goals.
