Journey Hospice, operated by Lutheran Social Ministries of New Jersey, seeks a Full-Time RN Case Manager to serve Hospice patients in Adams and Franklin Counties. The role includes overseeing a primary patient caseload, delivering skilled nursing care, and coordinating with the interdisciplinary team.

Requirements include graduation from an accredited RN program, PA RN licensure in good standing, and a valid driver’s license.

Qualifications

• Must be a graduate of an accredited registered nursing program.
• PA RN licensure must be in good standing.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care as prescribed.
• Perform initial and ongoing nursing assessments and develop IDT care plans.
• Educate and supervise patients, families, and staff regarding nursing care and hospice needs.
• Collaborate with IDT to ensure patient and family care goals are met.
