Journey Hospice has a rewarding and fulfilling Full-Time RN Case Manager position to serve our clients in Adams and Franklin Counties. This role offers a competitive salary and robust benefits!

The Hospice RN will oversee a primary caseload and provide skilled professional nursing care to patients and families as prescribed by the physician and agency policies, while collaborating with the IDT to ensure comprehensive care.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• PA RN Licensure by the PA State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation.

Responsibilities

• Oversee primary caseload of Hospice patients and provides skilled professional nursing care.
• Initial and comprehensive nursing assessment; collaborate to develop the IDT plan of care.
• Implements and reevaluates the plan of care to meet patient needs at home.
• Educates, supervises, and counsels patient, family, and staff on nursing care.
• Collaborates with IDT to ensure needs are met; supervises Hospice Aides/Homemakers.
