Job summary

Journey Hospice, part of Lutheran Social Ministries of New Jersey, seeks an RN Case Manager to serve clients in Adams and Franklin Counties. The role offers a competitive salary and robust benefits and is based in a Hospice setting with an interdisciplinary team.

The RN will manage a primary Hospice caseload, assess patient needs, and coordinate care with families and healthcare professionals in the home environment. Strong nursing, communication, and teamwork are essential.

Qualifications

• Graduate of an accredited registered nursing program.
• PA RN Licensure by the PA State Board of Nursing in good standing.
• Bachelor’s degree in Nursing preferred.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care.
• Perform initial and comprehensive nursing assessments and develop Hospice IDT plan of care.
• Educate, supervise, and counsel patient, family members, and staff on nursing care and patient needs.
• Collaborate with IDT to ensure patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
