Job summary

St. Gabriel's Hospice and Palliative Care in Dallas is seeking an RN Case Manager to perform admissions and provide bedside nursing care in patients’ homes, coordinating with the interdisciplinary team and families.

The role focuses on assessment, care planning, documentation, and on-call support with travel to patient residences. Ideal candidates hold an RN license, hospice experience, and strong communication skills to collaborate with physicians and referring agencies while meeting care

Qualifications

• One year RN hospice experience.
• WellSky Consolo experience preferred.
• Current state RN license and driver’s license.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria.
• Develop and document the plan of care with the Assessment.
• Provide nursing care with infection control measures.
• Coordinate with Interdisciplinary Team to meet patient/family needs.
• Communicate with attending and hospice physicians.
• Submit required paperwork in a timely manner.
• Participate in QAPI program and hospice in-service trainings.
• Meet productivity standards and provide on-call services.
• Communicate effectively with patients, families, and staff.
• Maintain reliable transportation and auto liability insurance.
