Job summary

Qualifications

• Two years RN experience in a clinical care setting with one year hospice required.
• Current State RN license and valid driver's license required.
• Knowledge of state and federal regulations with strong interpersonal skills.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document care plans in correlation with assessments.
• Provide nursing care with infection control measures.
• Coordinate care with Interdisciplinary Group/Team, patient, family, and referring agency.
• Communicate with attending physicians and hospice physicians involved in care.
• Timely submission of all required paperwork.
• Participate in hospice in-service programs and QAPI participation.
• Promote hospital philosophy and policies.
• Meet productivity standards and perform on-call responsibilities.
