Job summary

Infinity Hospice Care LLC is seeking a Registered Nurse Case Manager to perform admissions and provide nursing care in patients' homes. The role coordinates with the interdisciplinary team, patients, families, and referring agencies to ensure optimal care delivery.

The schedule is M-Th 5P-8A with on-call responsibilities, requiring travel to patient homes and offices within the service area. Applicants should have hospice experience and a valid GA RN license.

Qualifications

• Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse; Current driver’s license.
• Knowledge of state and federal regulations; good interpersonal skills.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria.
• Develop and document the plan of care.
• Provide nursing care with infection control measures.
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians.
• Submit required paperwork in a timely manner.
• Participate in hospice in-service programs and QAPI.
• Promote Hospice philosophy and policies.
• Meet productivity standards.
• Provide on-call services as assigned.
• Ensure effective communication with patients, families, and health care professionals.
• Maintain reliable transportation and auto liability insurance.
