BAYADA Home Health Care is seeking a registered nurse (RN) Case Manager, Hospice for a nursing job in Trenton, New Jersey. You will join a multidisciplinary team delivering palliative care in the home, with flexible scheduling and opportunities to travel within assigned territories.

Qualified candidates hold an active RN license and hospice experience is preferred. The role includes sign-on and benefits, with duties spanning medication administration, wound care, and ongoing patient monitoring.

Qualifications

• Active and current license to practice as a professional RN in PA.
• One year of hospice experience strongly preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Administration of medication, wound care, treatments and procedures, and comfort care.
• Monitoring clients’ conditions; reporting changes to Clinical or Client Services Manager.
• Occasional on-call after hours and/or weekends.
