Job summary

BAYADA Home Health Care is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to reliably serve patients in Montgomery and Bucks Counties, PA. You will be part of a multi-disciplinary team delivering palliative care at home, performing visits, administering medications, wound care, and documenting doctor orders.

The role requires PA RN licensure, hospice experience (preferred), and the ability to travel within designated territories.

Qualifications

• Active and current license to practice as a professional RN in PA.
• One year hospice experience strongly preferred.
• Previous home healthcare experience welcomed, but not required.
• Ability to travel to cases as assigned.
• Compassion, active listening, and strong written and verbal communication.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments and procedures, and provide comfort care.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up with and document doctors' orders.
• Occasional after-hours and weekend on-call duties.
