BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager for Hospice in Levittown, PA. You will join a multi-disciplinary team to provide compassionate home-based palliative care and coordinate services for patients in Montgomery and Bucks counties.

The role requires an active PA RN license and at least one year of hospice experience. Travel to assigned cases is required, with flexible scheduling and opportunities for ongoing training and professional growth.

Qualifications

• Active PA RN license.
• One year hospice experience preferred.
• Willingness to travel to cases as assigned.
• Strong written and verbal communication skills.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, perform wound care and treatments.
• Monitor client conditions and report changes to the manager.
• Follow up on doctors' orders and document actions.
• Occasional on-call after hours and weekends.
