Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing job in Bristol, Pennsylvania. You will make home visits to clients in designated territories, administer medications, monitor conditions, and document doctors' orders in a compassionate, patient-centered home care environment.

The role offers flexible scheduling with full-time/part-time options, a $10,000 Sign-On Bonus, and opportunities for training and advancement within a trusted home health

Qualifications

Active PA RN license.
Hospice experience strongly preferred.
Ability to travel to assigned cases.
Compassionate, reliable patient care advocate.

Responsibilities

Make home visits to clients in designated geographic territories.
Perform medications administration, wound care, treatments, and comfort care.
Monitor clients' conditions and report changes to supervisor.
Follow doctors' orders and document actions.
Occasional on-call after hours and/or weekends.
