Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for Roebling, New Jersey. The role involves making home visits, performing nursing duties, and coordinating with physicians and managers to deliver high-quality in-home hospice care.

The position offers flexible schedules, 1:1 hands-on training with virtual learning, and opportunities for continuing education and tuition reimbursement within a supportive care team.

Qualifications

• Active and current RN license in PA.
• One year hospice experience strongly preferred.
• Home healthcare experience welcomed but not required.
• Ability to travel to cases as assigned.
• Excellent communication and assessment skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform medication administration, wound care, treatments, and comfort care.
• Monitor clients' conditions and report changes to Clinical or Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Occasional on-call after hours and/or weekends.
