Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager for Hospice in Fort Washington, PA. You will be part of a multidisciplinary team delivering high-quality home-based palliative care, making regular home visits, and coordinating with physicians and other caregivers.

The role requires PA RN licensure, hospice experience preferred, and flexibility to travel within Montgomery/Bucks counties.

Qualifications

• Active and current PA RN license.
• One year hospice experience strongly preferred.
• Willingness and ability to travel to cases.
• Excellent written and verbal communication skills.
• Ability to deliver compassionate, reliable patient care at home.

Responsibilities

• Make home visits to patients in designated geographic territories.
• Administer medications, wound care, and treatments as prescribed.
• Monitor patient conditions and report changes to the care team.
• Document doctors' orders and care plans accurately.
• Coordinate with clinicians on-call as needed.
