Job summary
BAYADA Home Health Care in Yardley, PA is seeking a compassionate Registered Nurse (RN) Case Manager for Hospice to provide high-quality care at home. You will join a multi-disciplinary team delivering palliative care, making home visits, and coordinating patient needs.
The role requires an active PA RN license, hospice experience preferred, and willingness to travel within designated territories. Flexible schedules and a $10,000 sign-on bonus are offered to qualified candidates.
Qualifications
- Active and current license to practice as an RN in Pennsylvania.
- One year of hospice experience strongly preferred.
- Willingness and ability to travel to patient cases within assigned territory.
- Excellent communication and empathy when caring for patients and families.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Administer medications, wound care, treatments, and comfort care as prescribed.
- Monitor patients and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and document doctors’ orders accurately.
- Occasional on-call after hours and/or weekends.
