BAYADA Home Health Care in Horsham, PA is seeking a compassionate Registered Nurse (RN) Case Manager, Hospice to deliver 1:1 in-home care across Montgomery and Bucks counties.

Qualifications

• Active and current RN license in PA.
• At least 1 year hospice experience preferred.
• Home health care experience welcomed but not required.
• Ability to travel to assigned cases.
• Strong communication and assessment skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medications, provide wound care, treatments and comfort care.
• Monitor client conditions and report changes to supervisors.
• Follow up on doctors' orders and document care steps.
