Job summary
BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing role in Warminster, Pennsylvania. You will join a multidisciplinary team to provide compassionate palliative care at patients' homes across Montgomery and Bucks Counties. Responsibilities include home visits, administering meds, wound care, monitoring conditions and documenting orders. A PA RN license and hospice experience are preferred; travel is required with on-call after hours and weekends.
Qualifications
Active and current PA RN license.
One year hospice experience strongly preferred.
Ability to travel to cases as assigned.
Compassionate, attentive and reliable care.
Excellent verbal and written communication.
Responsibilities
Make home visits to clients in designated geographic territories.
Administer medications, wound care, treatments, and comfort care.
Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
Follow up with and document doctors' orders.
Occasional on-call after hours and weekends.
