BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a nursing job in Warrington, Pennsylvania. This role involves making home visits, administering medications, wound care, and ensuring effective care coordination across Montgomery and Bucks County.

The ideal candidate holds a PA RN license, with hospice experience preferred, and willingness to travel to assigned cases. We offer a sign-on bonus and comprehensive benefits as part of a dedicated home health team.

Qualifications

Active and current license to practice as a professional registered nurse (RN) in the state of PA.
One year of hospice experience strongly preferred.
Previous home healthcare experience welcomed, but not required.
Ability to travel to cases as assigned
A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.

Responsibilities

Making home visits to clients in designated geographic territories.
Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Occasional on call after hours and/or weekends
