Qualifications

• Active RN license (Pennsylvania) required.
• Hospice or home health nursing experience preferred.
• Excellent communication and organizational skills.

Responsibilities

• Perform patient admissions and nursing assessments.

• Coordinate with home health aides, social workers, and chaplains.
• Educate patients and families on disease progression and symptom management.
