Job summary

Journey Hospice in York County is seeking a full-time RN Case Manager to oversee a primary caseload of hospice patients, families and caregivers. You will perform comprehensive nursing assessments, develop and adjust the plan of care with the IDT, and supervise hospice aides while teaching families and coordinating care.

Requires RN licensure (NJ) with CHPN preferred; strong communication and teamwork are essential. Journey Hospice offers a competitive salary and robust benefits.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.

Responsibilities

• Oversee a primary caseload of hospice patients and provide skilled professional nursing care.
• Perform initial and ongoing nursing assessments and develop IDT-driven plans of care.
• Teach, supervise, and counsel patients, families and staff on nursing care.
• Collaborate with the IDT to ensure patient and family care needs are met.
• Supervise Hospice Aides/Homemakers and ensure quality care.
