Job summary

Journey Hospice in York County is actively seeking a full-time RN Case Manager. The role covers a primary caseload of Hospice patients, delivering skilled nursing care to patients, families, and caregivers in accordance with physician orders and regulatory standards.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure in good standing.

Responsibilities

• Provide skilled professional nursing care to Hospice patients and families.
• Conduct initial and ongoing nursing assessments and identify patient problems.
• Collaborate with the Interdisciplinary Team (IDT) to develop and revise the plan of care.
• Educate, supervise, and counsel patients, family members, and staff on nursing care.
• Coordinate with IDT to ensure patient and family needs are met; supervise Hospice Aides/Homecare staff.
