Journey Hospice in York County seeks a dedicated RN Case Manager to oversee a primary caseload and deliver skilled nursing care in the home setting. You will perform comprehensive assessments, collaborate with the interdisciplinary team, and ensure care plans reflect patient needs and family preferences.

The role requires an RN license, and a Bachelor’s degree is preferred; CHPN is a plus. Competitive salary and robust benefits are offered with full-time hours.

Qualifikationen

• Graduate of an accredited registered nursing program.
• Bachelor’s degree preferred.
• Certified Hospice and Palliative Nurse (CHPN) preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation.

Aufgaben

• Perform initial and ongoing nursing assessments and identify problems.
• Develop, implement, and reevaluate the plan of care with the IDT.
• Provide skilled nursing care to patients and families at home.
• Supervise Hospice Aides/Homemakers and counsel family members.
• Collaborate with IDT to meet patient and family goals and comply with regulations.
