Job summary

Journey Hospice seeks a qualified RN Case Manager to oversee a primary Hospice caseload in York County. You will provide skilled nursing care, complete initial and ongoing nursing assessments, and collaborate with the Interdisciplinary Team to develop and revise the care plan for patients and families.

Requirements include graduation from an accredited RN program and NJ licensure; a Bachelor’s degree in Nursing and CHPN certification are preferred.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor's degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid drivers license and reliable transportation that is insured in accordance with NJ state requirements.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled nursing care to patients, families and caregivers as prescribed by the physician and in compliance with Hospice standards.
• Conduct initial and comprehensive nursing assessments and identify problems; reassess and revise the plan of care with IDT.
• Teach, supervise, and counsel patients, families, and staff regarding nursing care and patient needs.
• Collaborate with IDT to ensure all patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
