Job summary

Journey Hospice is seeking a dedicated RN Case Manager to join our York County team on a full-time basis. You will oversee a primary caseload of hospice patients, provide skilled nursing care, and coordinate with families and the Interdisciplinary Team to deliver compassionate care.

Applicants must graduate from an accredited RN program, be licensed in New Jersey, and ideally hold CHPN certification. A valid driver's license and reliable transportation are required.

Qualifications

• Must be a graduate of an accredited registered nursing program.
• Bachelor's degree in Nursing preferred.
• NJ RN Licensure in good standing is required.
• CHPN certification preferred.

Responsibilities

• Oversee a primary hospice patient caseload and provide skilled nursing care as prescribed by the physician.
• Conduct initial and ongoing nursing assessments; revise the plan of care with the IDT.
• Educate, supervise and counsel patients, families and caregivers about nursing care and home settings.
