Job summary

Journey Hospice is seeking a Full-Time RN Case Manager to oversee patient care in Atlantic County. This salaried position offers $90,000 to $95,000 annually based on experience. Responsibilities include managing caseloads, providing nursing assessments, and developing care plans.

The ideal candidate will have a nursing degree, NJ RN Licensure, and preferably CHPN certification. The role includes a range of benefits such as medical and dental insurance, flexible pay, and a 403(b) retirement plan.

Qualifications

• Minimum of one (1) year general nursing preferred, experience with hospice/home health preferred.
• Ability to work within an interdisciplinary setting.
• Understanding of hospice philosophy and principles of death/dying.

Responsibilities

• Oversee primary caseload of hospice patients providing skilled professional nursing care.
• Implement and revise plans of care relative to patient’s nursing needs.
• Provide supervision to hospice aides/homemakers.
