Job summary

Lutheran Social Ministries of New Jersey is seeking a dedicated Hospice RN Case Manager to join our York County team. The role manages a primary caseload of hospice patients, delivers skilled nursing care, and coordinates with families and caregivers.

You will perform initial and ongoing assessments, develop and revise the care plan with the Interdisciplinary Team, and educate patients and staff. A NJ RN license and hospice/home-health experience are preferred; CHPN certification is a plus.

Qualifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation that is insured in accordance with NJ state requirements.

Responsibilities

• Oversee primary caseload of Hospice patients and provide skilled professional nursing care.
• Perform initial and comprehensive nursing assessments; reassess and revise the plan of care with the IDT.
• Teach, supervise, and counsel patients, families, and staff on nursing care and patient needs.
• Collaborate with IDT to ensure all patient and family care needs are met.
• Provide supervision to Hospice Aides/Homemakers.
