Samaritan in New Jersey is seeking a Hospice Nurse to oversee the quality and expert care of assigned hospice patients in a full-time role. You will perform patient assessments for comfort and symptom relief, establish individualized plans of care, educate patients and families, obtain physician orders, coordinate home visit schedules, and collaborate with social workers and IDT members as needed.

Qualifications

• Minimum of two years of nursing experience.
• NJ RN license required; BSN preferred.
• Bilingual is a plus.

Responsibilities

• Perform patient assessments for comfort and symptom relief.
• Create and implement an individualized plan of care.
• Assess education needs for patients and families and provide information.
• Obtain physician orders to achieve symptom management.
• Triage and arrange home visit schedules with efficient routing.
• Refer to IDT members and coordinate with social workers.
