LSMNJ is seeking a dedicated RN Case Manager for Hospice care, based in York County. The role involves coordinating patient assessments, leading the Interdisciplinary Team, and ensuring high-quality care for patients and families.

The ideal candidate holds an accredited RN license (NJ) and preferably CHPN certification. A valid driver’s license is required, and the position includes competitive salary and robust benefits.

Formación

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Oversees Hospice patient caseload and provides skilled nursing care to patients, families and caregivers.
• Performs initial and comprehensive nursing assessments and develops the IDT plan of care.
• Implements and reevaluates the plan of care to meet patient needs.
• Teaches, supervises, and counsels patients, families, and staff.
• Provides supervision to Hospice Aides/Homemakers and collaborates with IDT.
