Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care, drawing on community resources to meet the needs of patients and families in their homes and communities.

The RN will perform comprehensive assessments, develop and revise care plans, administer medications, and educate patients and families. Collaboration with the interdisciplinary team, on-call duties, and ensuring a safe, supportive care environment are essential components of this role.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed to practice in good standing.
• One year of professional nursing experience within the last three years preferred, or a baccalaureate in nursing with two years’ experience in public health, home care, or hospice.

Responsibilities

• Performs comprehensive assessments of patient and family to determine hospice needs.
• Develops and revises plan of care based on nursing diagnoses and patient status.
• Administers medications and treatments as prescribed.
• Counsels patient and family on nursing and related needs and provides health instructions.
• Acts as Case Manager and coordinates patient care across the hospice team.
• Participates in on-call duties and ensures resources and equipment are available.
• Maintains accurate clinical notes and collaborates with physicians and the interdisciplinary group.
