Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings, collaborating with physicians, social workers and other IDG members to develop and refine care plans. You will perform comprehensive assessments, deliver skilled nursing interventions, and educate families as part of a holistic approach to end-of-life care.

You will provide education to caregivers, ensure timely documentation, and participate in on-call duties while upholding regulatory

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, acute care, oncology, or long-term care.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Lead patients and families through the hospice journey as primary contact.
• Deliver and document skilled nursing care per Plan of Care in home settings.
• Perform initial and ongoing assessments and adjust care.
• Collaborate with IDG to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe care and practical skills.
• Communicate changes in patient condition to the care team.
• Maintain accurate clinical documentation and infection-control standards.
• Support continuity of care across home health, palliative, and hospice transitions.
• Participate in on-call and patient support activities.
