Job summary

Wellstar Health System in Georgia is seeking a Hospice RN Case Manager to coordinate care for terminally ill patients and support their families. You will assess, plan, implement and evaluate care in collaboration with the Hospice team, physicians and social workers, and educate caregivers on symptom management and medications.

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan.
• Maintains ongoing contact with patients/families (phone calls, visits).
• Educates caregivers on disease processes, symptom management, medications, and nutrition.
• Manages medications ensuring availability, correct administration, safe storage, and DEA guidelines.
