Job summary

Wellstar Hospice is seeking an RN Case Manager to lead clinical care for terminally ill patients and support their families. You will coordinate with physicians, social workers, chaplains, and volunteers to deliver compassionate, evidence-based care enabled by electronic records and ongoing assessments.

The role requires an RN license, BLS certification, and state driver’s license. Prior hospice or home care experience is preferred, with the ability to educate families and manage complex symptom

Qualifications

• Minimum education as stated: Associate degree or Diploma in Nursing; Bachelor’s preferred.
• Proven clinical assessment and care planning abilities.
• Knowledge of disease processes across age groups and hospice care.

Responsibilities

• Provides ongoing patient care per interdisciplinary plan.
• Maintains contact with patients/families via visits and calls.
• Educates on disease processes, symptoms, medications, and nutrition.
• Assesses symptoms and medication effectiveness, documents progress.
• Facilitates transfers to appropriate care settings when needed.
• Ensures safety and compliance with care standards, including DEA guidelines.
