Job summary

Wellstar Health System in Georgia is seeking a Hospice RN Case Manager to provide comprehensive nursing care for terminally ill patients and support their families. The role involves coordinating with the primary physician, Hospice Medical Director, and the interdisciplinary team to implement and adjust care plans.

The RN will educate families, monitor symptoms, document care activities in the EMR, and coordinate transfers to appropriate facilities as death approaches.

Qualifications

• Associates Degree in Nursing or Diploma in Nursing, or BSN preferred.
• Georgia RN license required; BLS certification required.
• 2 years hospice or acute care experience preferred.
• Must have valid Georgia driver’s license and access to a personal vehicle.

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan.
• Maintains contact with patients and families via visits and calls.
• Educates on disease processes, symptom management, medications, and nutrition.
• Manages medications and ensures timely administration and proper storage.
• Assesses symptoms and communicates changes to the physician.
• Collaborates with the interdisciplinary team and transfers to appropriate care settings as needed.
