Job summary

Wellstar Health System is seeking a Hospice RN Case Manager to assess, plan, implement, and evaluate care for terminally ill patients in collaboration with the Hospice Medical Director and interdisciplinary team. The role includes coordinating with clinicians, social workers, chaplains, and volunteers while providing compassionate physical and psychosocial support to patients and families.

A valid Georgia RN license and BLS are required, with mileage reimbursement for use of a personal vehicle.

Qualifications

• Active RN license in Georgia (or multi-state compact)
• BLS certification
• Driver's license and private vehicle insurance
• CHPN certification (preferred)

Responsibilities

• Provides ongoing patient care according to the interdisciplinary care plan.
• Maintains ongoing contact with patients and families (calls and visits).
• Educates about disease processes, symptom management, medications, and nutrition.
• Assesses symptoms and medication effectiveness; communicates with physicians.
• Coordinates care with Interdisciplinary Team and facilitates transfers as needed.
• Documents plan of care, progress notes, and ICD-10 coding.
• Ensures safety, monitors infection control, and reports quality concerns.
