Job summary

Wellstar Health System is seeking a Hospice RN Case Manager in Georgia to provide holistic nursing care to terminally ill patients and support to families. The role collaborates with physicians, social workers, chaplains and an interdisciplinary team to coordinate care and ensure comfort and quality of life.

The RN Case Manager conducts assessments, plans interventions, educates families, and ensures documentation compliance while guiding care transitions and advocating for patient needs.

Qualifications

• Possesses strong assessment and clinical judgment across diverse patient groups.
• Demonstrates effective communication with patients, families and team members.
• Maintains accurate electronic medical records and patient documentation.

Responsibilities

• Provides ongoing patient care per interdisciplinary care plan.
• Maintains contact with patients and families via calls and visits.
• Educates on disease processes, symptom management, medications, and nutrition.
• Manages medications and DEA considerations with proper storage and disposal.
• Assesses symptoms, monitors response to treatment and documents changes.
• Facilitates transfers to appropriate care facilities when needed.
• Ensures patient safety and caregiver support; reviews and updates care plans.
• Participates in team meetings and family conferences; acts as clinical resource.
